The defect
The retaining washer may become loose from the bolt that connects the brake pedal with the bearing bracket. Should the retaining washer become loose, the bolt may slip out/work loose through continued use of the brake pedal in the course of vehicle operation.
The hazard
This could result in the brake lights remaining on even while the brake pedal is not applied. The traffic behind the vehicle will be unable to identify whether your vehicle is actually braking. This may lead to an accident causing injury or death of vehicle occupants and/or other road users.
The remedy
